It is in Dutch but is a standard form so you should have no problems, if you do, you the contact page and ADOforExpats will help you out.
Complete the application form in block capital letters then take it to the ticket department along with a valid ID to the Kyocera Stadium. When submitting the application, the required biometric photograph must be taken. All applications are processed in order of receipt.
The sale of the Half Season tickets will start on Tuesday, December 4. From this date, it is possible to enter the Kyocera Stadium get your ticket and guarantee your place in the stadium.

NOTE: Creating a biometric photograph takes some time. Please therefore take into account possible long waiting times.
By requesting the ADO Den Haag Half Season ticket, the applicant must agree to the “Terms Provision (Season) Club Card”. Furthermore you agree with the rules of procedure of the KNVB and the house rules for home and away games of ADO Den Haag. These are available at the Ticketing Department.

Loss or theft of the (Season) ticket
If your (Season) ticket is lost or stolen, you can return to the Ticketing Department at the Kyocera stadium and apply for a new one..
If you report a theft to the police, then the club charge you nothing for a new card.. Otherwise there is a charge of €15, 00 administrative costs.

Advantages of a Half Season Club Card
The purchase of a Half Season ticket offers many benefits. Thus, the advantage of a Half Season ticket is a saving of up to € 130, 00 compared to single ticket sales. Furthermore, Season ticket Holder are assured of a place in the stands and you do not run the risk of missing the top matches against FC Twente and Feyenoord.
In addition to these advantages the Half Season ticket also offers the holder more advantages;
1. The Season ticket is valid for 5 years. After that, a new biometric photo can be taken. The new hardware card is available free of charge by post.
2. A Season ticket gives priority to regular ticket sales for away games.
3. A Season ticket Holder receives at least 5% discount on purchases in the official Fan shop of ADO Den Haag.
4. As a half season ticket holder you get free membership of the Fan Club ADO Den Haag, the official supporters club of ADO Den Haag (normally 20 Euros per year)
5. Free delivery of the fanzine four times per year.
6. Season ticket holders have the option at certain matches to purchase up to 4 tickets via Ticketbox outlets or by the online ticketing option.
